Materials, Techniques and Equipment

We are proud to say we use the highest quality materials and equipment at Smile Cliniq. Where possible we use materials and equipment independent tested by Reality 5 star and Dental Advisor. We use the latest technique taught by world leading clinicians. Suppliers include, Dentsply, GC, 3M, 3i Biomet, Triodent and Heraeus Kulzer, so patient can rest assured their Oral Health is being maintained with the very best of materials, equipment and techniques.


Diagnosis

New patient comprehensive examination

The dental examination consists of a thorough check up of the teeth, gums and soft tissues. We also check the submandibular area and temporomandibular joint apparatus for any signs of pathology.  Also included is an Oral Cancer screen, which we do for every patient.  Any necessary small digital x-rays are also included. We use the Digora digital radiograph system which allows radiographs to be taken at low x-ray dosage, and produces high quality, digital radiographs in a matter of seconds.

Routine dental examination

Full case assessment by a specialist (orthodontist, periodontist, prosthodontist)

Our highly experienced specialists will discuss your concerns and answer any questions you may have. They will then perform a full assessment and explain their findings and give you your treatment options. Lastly they will formulate a customised treatment plan showing all costs and appointment scheduling. This will be given to you as a written report.

Small digital radiographs (70% less radiation than non-digital)
We use the Digora digital radiograph system which allows radiographs to be taken at low x-ray dosage, and produces high quality, digital radiographs in a matter of seconds.

Large radiographs (Panoramic)

We use the Kodak digital radiograph system which allows radiographs to be taken at low x-ray dosage, and produces high quality, digital radiographs in a matter of seconds. These radiographs maybe used to assess bone levels or diseases of the jaw bone. It can also be used before extraction to assess root anatomy or prior to implant placement.


Periodontal treatment

Hygiene appointments

The hygiene appointment, otherwise known as a scale and polish, consists of the removal of plaque and calculus above and slightly below the gum line. Stains on teeth due to diet and smoking are also removed using the ultrasonic scaler and hand instruments. Deep stains may need a second appointment for complete removal. Once the plaque, calculus, and staining is removed from the teeth, a polish of the teeth is performed using prophylactic paste to smoothen and bring shine to your teeth.


Fillings (non mercury)

Composite

Composite fillings are white fillings.  We use high quality, strong, reliable and aesthetic composites such as Venus from Hereus Kulzer, and G-ænial from GC, with low shrinkage properties (to prevent post-operative sensitivity). A particular shade is selected to match the natural shade of your teeth.  Often two or more shades maybe used, to build up the dentine and then the enamel shades (no tooth is one colour, instead the tooth near the gum is darker, whereas the tooth at the biting surface is more translucent and lighter). The procedure of a composite filling is to firstly to isolate the tooth adequately, from the oral environment, sometimes using a rubber dam material to protect the composite from moisture.  We then remove the decay inside the tooth or the old mercury filling, and shape the cavity. An etchant gel is applied which roughens the surface of the tooth on a microscopic level which promotes adherence (micro mechanical retention) of the composite to the tooth. A good quality bond is then applied to allow the filling to “bond” to the dentine. The selected shade of composite is placed inside the cavity in small increments, curing each time with a blue light (around 450nm wavelength) to initiate the polymerisation reaction. Incremental build up is important to minimise shrinkage and stresses on the tooth, caused by the setting of the composite.  The restoration is finally shaped and polished at the surface and margins to allow perfect bite and prevent food trapping.

Glass Ionomer

Another white restoration used at Smile Cliniq is a type of glass ionomer filling material called Fuji IX Extra. The beneficial characteristic of this material is that it sticks to the tooth surface well and also releases fluoride.  Fuji IX Extra compared to other glass ionomer materials releases much more fluoride (upto 5 times more). In deep restorations a Fugi IX filling maybe placed, underneath composite restorations to protect the pulp (nerve tissues) and minimise the possibility of future problems, such as root canal treatments.

All occlusal (bite surface) restorations (fillings) have a 5 year guarantee unless otherwise specified.  Buccal restorations (side of tooth) have a 2 year guarantee.


Crowns

Crowns

Crowns are custom made restorations that fit over the whole tooth, protecting it and providing a long term strong, aesthetic cover to the treated tooth.

Various types of crowns are offered at Smile Cliniq, and their cost depend on the material of choice, time taken, complexity of work and laboratory used.

Gradia – White

Resin based material which chemically bonds to tooth, thus less drilling is required.

The GC GRADIA family is a unique light-cured composite system for indirect restorations such as crowns, bridges, inlays and veneers and all types of direct restorations whether it’s in the anterior or the posterior region.

With the GC GRADIA family, you can handle all your aesthetically demanding restorations easier and quicker. Due to the remarkable shade’s adaptation to the surrounding tooth structure, outstanding natural looking aesthetic and long-lasting restorations can easily be achieved.

The GC GRADIA family consists of GC GRADIA, the laboratory system for making high-end aesthetic crown and bridge work that has proven itself for over 5 years now.

Based on this material GC GRADIA gum shades is designed to reproduce the natural gingiva by means of basic or multiple layering techniques.

The different members of the GC GRADIA family are fully compatible with each other, delivering unlimited possibilities for invisible restorations.

EMAX

Ivoclar Vivadent’s patented lithium disilicate material is truly a revolution for the dental industry. Never before has a material been able to combine high strength, high esthetics and ease of use into one product, until now.
IPS e.max is a lithium disilicate glass ceramic that has optimized translucency, durability and strength for full anatomical restorations. Due to the use of new technologies and optimized processing parameters, IPS e.max lithium disilicate has evolved beyond previously available lithium disilicate ceramics. Indeed, compared to other glass-ceramics, demonstrates a strength that is 2.5 to 3 times higher. IPS e.max lithium disilicate restorations exhibit superior durability featuring 360-400 MPa of flexural strength. When fabricated to full-contour, the monolithic structure is the most robust ceramic system tested to date. The opalescence, translucency and light diffusion properties of IPS e.max lithium disilicate were all designed to replicate natural tooth structure for beauty and undetectable restorations.

All porcelain (Emax is our preferred indirect restoration) – White and incredibly aesthetic, often indistinguishable from natural teeth. Even stronger than Gradia and is also capable of chemically bonding to the tooth. Produces a very natural result.

Porcelain Bonded to Metal

White porcelain bonded to a metal sub-structure. The metal gives strength whilst the porcelain gives the white colour. Not as natural looking as emax and gradia crowns. The metal substructure may be non – precious or precious metal depending on the patient’s choice. Precious metal is generally preferable as it is more biocompatible with the gingival (gums) leading to less inflammation and is less prone to corrosion.

Gold – Metal crown

Very strong and durable. Has been a successful restoration for many decades. Has better wear characteristics than natural tooth enamel and therefore is very effective on back teeth , especially in people who grind their teeth. Also allow minimal tooth preparation, thus preserving more tooth structure.

Zirconia

Zirconia is the stongest crown material on the market today.
The Zirconia material typically used by most manufacturers is atetragonal polycrystalline zirconia, partially stabilized with yttrium oxide. Zirconia materials typically have strengths of over 900 MPa.

When preparing a tooth for a crown, bridge or veneer, it is important to obtain an accurate impression so that the restoration fits perfectly to the tooth, giving ultimate aesthetics, durability and comfort. At Smile Cliniq we use high quality impression materials from leading manufacturers such as Heraeus Kulzer.

The Fexitime Dynamix is released from a fast automated mixing machine. Its precise dosage as well as thorough mixing of material limits the possibility of retaking the impressions, and therefore less chairtime for the patient.
Along with the Flexitime Dynamix, we use the provil novo light body silicone material to obtain even more accurate impressions that replicate the tooth structure to a fraction of a millimetre. Use of such high quality material, means a high quality crown, veneer or bridge can be made with a perfect fit.

In certain cases (full stable occlusion) our dentists may use a novel impression tray called a triotray from a company called triodent. This tray allows impressions of both upper and lower quadrants to be taken at once as well as recording the bite at the same time. This reliable product allows as single impression to be used to construct the restoration instead of two separate impressions, and therefore reduces the number of times the patient has to have an impression taken. It also avoids the need for a full upper arch impression which some patients exhibit the initiation of the unpleasant GAG reflex. The triotray being made of metal, is stronger and less flexible than plastic trays, hence improving accuracy.

Veneers (Composite)

Composite veneers are usually performed by the dentist in one session. Multiple shades of composite are often required to match the changing shade gradient and translucency of the natural tooth. The shape of the composite veneer can be shaped to match the shape of the comparative existing tooth, or redesigned and adjusted to your preference. They require much skill and time from the dentist in order to produce a natural looking result, but have the advantage of eliminating lab costs and return appointments. However, although they can look very natural they are not usually as aesthetic as porcelain veneers. We use high quality, strong, reliable and aesthetic composites such as Venus from Hereus Kulzer, and G-aenial from GC.

Veneers (Porcelain)

Porcelain veneers are highly aesthetic, custom made ceramic facings that are designed to match your natural teeth but also incorporating your cosmetic preferences into it. Patients are welcomed to visit the ceramist where he will discuss with you how exactly you want the veneer to look, including the shape, shade, thickness, and natural features such as translucency and surface characteristics for porcelain veneers. Each person’s teeth look different from another. Some are more translucent, some have thin lines running down the length, and others have unique markings and a variety of shades. Therefore, making porcelain look like a natural tooth requires the skill of a highly trained ceramist. At Smile Cliniq we are dedicated in addressing the patient’s needs, and offer the highest quality of materials and level of service possible.

Bridges

Bridges are made in similar process to crowns, except that it consists of one or more units joined to the crown. Maryland bridges are also available in certain circumstances, which allows minimal preparation of the tooth (i.e. no need for a full crown preparation), thus preserving tooth structure.

Dentures

Our dentures are fabricated with high quality award winning life like, natural strong Engima teeth from Schottlander. In 2004, Schottlander were honoured to receive the Queen’s Award for their Enigma Teeth and Denture System. The Queens Award for Enterprise in the innovation category was bestowed on Schottlander in 2004 for their Enigma Teeth and Denture System. To quote the Queen’s Award Office “A Queen’s Award is the highest honour that can be bestowed on a UK business. To have a Queen’s Award is to be acknowledged as the best in your field”. Engima denture teeth have 27 different shapes of teeth available, with 17 different shades, so you really get the exact smile you wanted.


Implants

Single tooth (including abutment, surgical prep. and crown)

Implants performed at Smile Cliniq are performed by Dr Kaher, Mr MacLellan and Dr Pelekos. All with certified post graduate training in the placement of implants. The procedure is performed in a near sterile environment where the surgery is disinfected as much as possible and set out in a similar manner to a hospital environment. Clinicians are also scrubbed up, as in a surgical procedure in a hospital. This is important as it reduces ambient bacterial levels and hence reduces the chances of implant failure. Please note: all implants carried out at Smile Cliniq are carried out under Surgical conditions unlike other dental surgeries and there is no extra charge for this.

Implants used at Smile Cliniq are Implant Direct and Biomet 3i implants. The Biomet implant is the third largest implant company in the world, and an impressive amount of extensive research (over 100 scientific publications) and technology has been put into producing the implant. The Osseotite® Implant by Biomet features their acid etch surface which has been proven to encourage bone cells to grow around the implant. It also allows platform switching, producing much better soft tissue results once the final restoration is placed. Nevertheless, both implants systems have a proven high success rate.

Implants are made from titanium and are extremely strong, and once successfully placed, should last for many many years. Indeed Implants have now been in place in some patients for over 30 years.

Although the placement of an implant may seem daunting to some, it is actually much easier to carry out than some extractions, and the patient is always completely numb so that they do not feel any pain during treatment. We use highest quality instruments as well as thoroughly researched, tried and tested materials and methods to ensure our patients receive the best treatment possible.


Orthodontics

Invisalign

Invisalign is a clear, removable orthodontic appliance and its popularity has grown significantly due to its near invisible property and the simplicity of the treatment. Impressions are taken of the upper and lower teeth, and the occlusion (bite). These impressions are sent off to the Invisalign laboratory abroad where they will use the impressions to formulate a CAD-CAM treatment plan called Clin Check. The Clin Check will tell you how many aligners are required, how long the treatment will take and what your teeth will look like at the end of treatment. This will allow you to visualise the results of your treatment before commencing treatment, and therefore gives you a chance to make alterations before committing to the treatment.

Inman Aligner

The Inman Aligner is a cost-effective removable orthodontic appliance that moves teeth in a fraction of the time and cost of other systems. It aligns crowded or protruding anterior teeth with two opposing spring loaded aligner bars.

At Smile Cliniq we use high quality impression material from leading manufacturer such as Heraeus Kulzer. We use provil putty along with provil novo light body silicone material amongst other materials to obtain the most accurate impressions that replicate the tooth structure by a fraction of a millimetre. This in turn allows the lab to produce an aligner specific to your case.

After fitting your Inman Aligner, we review you every two to three weeks to assess the movement, create space when required to allow movement, and tighten the aligner to ensure continuous force of the bars.

We always recommend retainer after your teeth are straightened. This holds the teeth in their newly straightened positions. The retention maybe fixed or removable.

Fixed Orthodontics (with orthodontist)

Our highly experienced in-house Orthodontist Dr Pasqualicchio uses white ceramic brackets from the leading dental company 3M unitek on the upper teeth instead of the silver metal brackets. The price includes the cost of all materials and appointments including adjusting and review appointments. You will see the orthodontist around every 6 weeks to adjust and tighten the brace, until you finish the treatment with much straighter teeth.

With all types of orthodontic treatments, fixed or removable retention is strongly recommended to hold the teeth in their newly aligned positions going forward.


Root canal treatments

Per canal (using single use instruments)

Root canal treatment is one of the most challenging treatments for a dentist to perform. There are many risks and possible causes of problems when performing a root canal treatment, and accurate measurements, correct instrumentation and moisture control is vital to ensuring a good result.

At Smile Cliniq, we make sure that the risk of failure is minimised by using high tech equipment to locate the ends of the roots, ensuring the filling reaches the whole length of the root canal.
We also use specially designed fibre optic lights that the dentist can use with their magnifying loupes (spectacles), for a much clearer vision of the very small root canal openings.
A variety of different sized, sterile files are used to smooth and shape the canals.

Maintaining a moisture-free sterile environment when performing a root canal treatment is important as this prevents any bacteria in the mouth from entering the canals. To do this, a rubber dam maybe placed on the tooth to be treated. This is a thin sheet of rubber held on by a metal clasp around the tooth. Sodium hypochlorite, chlorhexidine and occasionally iodine are used to disinfect the canals to destroy any remaining bacteria inside the canals.

We also use highly rated, costly and modern equipment when placing the filling material – the hot shot and hot tip from smart endodontics.

The HotShot is a cordless backfill obturation device that fills the canals. The thin and flexible needles provide easy access to even very thin canals, ensuring fuller obturation lowering the risk of re-infection.

The HotTip is a cordless warm vertical compaction device, and comes in a variety of sizes to accommodate different sized canals.
Use of these equipments reduces chair time for the patient and allows the dentist to carry out the treatment smoothly and easily, with better results allowing for a minimally stressful experience.


Extractions

Simple
Surgical

Cost of extractions depends on the difficulty of the extraction. The instruments we use for extracting teeth at Smile Cliniq are designed to cause minimal bone trauma (and hence post operative pain), which will help maintain bone levels that maybe required for possible future treatments such as implants and dentures, or simply to keep a fuller jaw contour.


Cosmetic Treatments

Home bleaching

An impression of your teeth is taken in the surgery which is sent to the lab. A custom made bleaching tray is sent back to Smile Cliniq which is fitted on the second appointment. Along with the tray, 2 syringes of  take-home bleach are given along with a tray box and a sensitive toothpaste sample. This is enough for 4-6 applications on each upper and lower teeth, and is worn for up to 4 hours a day. Take-home whitening gradually whitens teeth, by holding the tooth whitening gel against the teeth for a prolonged time and therefore gives a stable result.  It is also more effective for deep lying stains within the tooth.

In-chair bleaching

In-chair whitening is when whitening is performed in the surgery, and uses a stronger percentage of whitening bleach. The cheeks and gums are protected using a retractor and dam, and a layer of bleach is applied to the teeth. The bleach is then activated by a blue light and left to work for 10 minutes. After 10 minutes, a new layer of bleach is applied and the process repeated for 2 to 3 times.
In-chair whitening gives an instant whiter result.


General

Sedation

During dental sedation, an Anaesthetist will carry out the sedation. Ranjana Khiroya is our anaesthetist with 36 years of experience, and will ensure that the patient is feeling relaxed and happy while monitoring the blood pressure, oxygen saturation in the blood, and the pulse. The dental treatment is carried out by the dentist who would have previously talked to the patient in the consultation and examination, with the aneasthetist and nurse keeping a close eye on the patient during sedation.
Once the treatment is finished, the patient is left to relax and is taken home by a friend or relative.

Anti Snoring devices

We at Smile Cliniq spend time taking a thorough history and carrying out diagnostic tests as to the cause of your snoring. After this, we recommend the appropriate treatment.
Possible treatments can include nasal decongestants, antihistamines or provision of an anti-snoring device. These devices are custom made for each individual patient. The process is very simple involving the dentist taking moulds of the upper and lower jaw. These moulds are then sent to our lab where the appliance is constructed. This appliance is custom made for you and is not one of the readily available “generic” devices, where one size fits all. You will wear it when you sleep and it positions the mandible in such a way that it promotes breathing through the nose and reduces the amount of obstruction caused by the soft palate encroaching on the airway. Most patients take very little time to get used to this.
